PARTICIPATION WAIVER FORMS Due to COVID-19, the following waiver forms are required for use by both student participants and staff / volunteers. Both forms are PDFs and can be filled out on-screen. Please complete the form, print it, and then sign it before turning it in. Athletics forms should be turned in to the Athletics Department, and forms for non-athletics activities should be submitted to the respective building principal.
